Job Description

Actify Home Care is a therapist owned and operated Medicare Home Health agency in
Broward County, providing both home health medical services and home care non-medical
services, to support our patients to live safely and comfortably at home with the support they
need, and give their families peace of mind.
For our patient in Sunrise, FL we are putting together a dedicated team of ENGLISH
SPEAKING per-diem Registered Nurses with Home Health experience, to give care to
our patient needing immediate private duty skilled nursing care, in 8-hour shifts.
Option for 8-hour shifts, minimum 24 hours, up to 40 hours/week - days, evenings,
weekends.

You work independently, but are not alone. We have dedicated and knowledgeable
management and office staff available to assist you 7 days per week. Our staff at Actify
handles all credential management, authorization tracking, DME processing, referral
processing, and agency billing, so that you can focus on using your skills for
compassionate care to develop your patient relationship, and enjoy the satisfaction of
the family’s appreciation.
Key Qualifications:
English speaking, reading, and writing
Reliable commuting transportation
Professionalism with compassion, joy, and great problem-solving skills
Experience with tracheostomy and feeding tubes
