The successful candidate will provide comprehensive emotional support, counseling, and guidance, ensuring that both patients and their loved ones receive the care and resources they need during this challenging time.

Key Responsibilities:

• Conduct thorough assessments of the psychosocial needs of patients and their families.
• Collaborate with healthcare professionals to develop and implement personalized care plans.
• Facilitate access to community resources and support services.
• Offer bereavement support, addressing emotional, financial, and social aspects of end-of-life care.
• Serve as an advocate for patients, ensuring dignity and comfort are prioritized.
• Coordinate treatments, services, and resources to improve patient quality of life.
• Provide counseling and referral services to help families cope with stress and hardships.

Qualifications:

• Master’s Degree in Social Work.
• Proven experience as a social worker, preferably in a hospice or healthcare setting.
• Strong interpersonal and communication skills, with an ability to navigate complex emotional situations.
• Reliable transportation is required.
