Job Summary : The High Fidelity Wraparound (HFW) Facilitator is responsible for utilizing the wraparound model to guide the development of a collaborative, strengths-based, and comprehensive service plan for children and families. This role involves overseeing the wraparound process, facilitating team development, and ensuring the delivery of services with the highest fidelity to the model. The HFW Facilitator will also provide consultation and education to service providers, monitor progress toward goals, and ensure all necessary data for evaluation is accurately gathered and entered. They will work closely with the team to complete strengths/needs assessments, identify service providers, engage natural supports, and ensure the successful implementation of the service plan.
