Job Summary : The High Fidelity Wraparound (HFW) Facilitator is responsible for utilizing the wraparound model to guide the development of a collaborative, strengths-based, and comprehensive service plan for children and families. This role involves overseeing the wraparound process, facilitating team development, and ensuring the delivery of services with the highest fidelity to the model. The HFW Facilitator will also provide consultation and education to service providers, monitor progress toward goals, and ensure all necessary data for evaluation is accurately gathered and entered. They will work closely with the team to complete strengths/needs assessments, identify service providers, engage natural supports, and ensure the successful implementation of the service plan.

Qualifications :

• Education : Bachelor’s or Master’s degree in social work, psychology, counseling, or a related field.
• Experience : Previous experience working with children, families, or in behavioral health is preferred. 
  • One year of experience providing TCM services to children/youth with SED and their families.
  • Willingness and ability to complete HFW Facilitator training.
  • Access to reliable transportation and a valid driver's license.
• Skills :
  • Strong facilitation, communication, and interpersonal skills.
  • An understanding of and experience with different child-serving systems, including education, behavioral health, child welfare, juvenile justice, health and others
  • Experience in case management and team coordination.
  • Ability to apply the wraparound model with high fidelity and ensure all team members are engaged and aligned with the service plan.
  • Skilled in assessing strengths, needs, and resources, and using that information to develop effective, individualized plans.
  • Knowledge of community resources and ability to connect families with appropriate services.
  • Experience working with and a genuine respect for families of children/youth who have complex behavioral health needs and a firm commitment to empowering families.
