HYBRID schedule! Expectation of meeting in-person once a week (Mondays) and working remotely for the remainder of the week. Subject to change, depending on the need.
We are seeking a bilingual English / Spanish ADS coordinator / Adults with Disabilities. Responsible for day-to-day activities that support the delivery of high quality, person-centered, outcome-based day services for adults with intellectual and developmental disabilities. Assists with the coordination of the intake process and assessments, as well as planning, scheduling and coordination of activities in alignment with individuals’ personal plan and within the scope of the service design. Supports service compliance with applicable standards and regulations.

M-F 8am -4pm
Brief Description :
Processing incoming / new referrals, inputting them into Therap (EHR). Contacting new referrals, explaining the referral process and next steps; fulfilling referrals.
Conducting interval check-ins with existing Tech Lending Library participants, ensuring needs are still being met and offering support when applicable.
Scheduling in-person and / or remote training for applicable participants, further developing training curriculum.
Collaborating with internal departments and other company associates during the referral process / while managing individual cases.
Maintaining communication with and updating outside agencies (Regional Centers) on referred participants.
EDUCATION :
Associate’s degree in Human Services or related field is preferred.
Possess and maintain valid CPR and First Aid Certifications.
EXPERIENCE :
Two (2) years of work related experience in Human Services, Adult Day Center / Services, or similar field.
Or the combination of experience, education, and training to perform the job successfully.
KNOWLEDGE, SKILLS, ABILITIES :
Knowledge of person-centered programming approach providing services to create purposeful and meaningful activities.
Ability to demonstrate actively listening, patience, and empathy in communicating with individuals that use the service.
